If you see the spinning dots, which might not be the case, windows should automatically try to rebuild the startup image after about the 4th failed boot.
Maybe plug in a USB or CD and see if it finds it as a backup, worked for the Windows 8 to 10 update on a windows machine. Apple killed most of the 'bless' service that would allow other EFI-based OSs to load from boot, hence why boot camp is set up while Mac OS is running. Before that, you could boot from a clean windows image and use the startup repair tool within the installer. No spinning dots. Just the blue 4x4 logo appears, blinks once (i.e. It is on, then very briefly gone, then on again), and then the screen goes black. It happens quickly.
I will try with a repair DVD (standard Windows 10). I will also try with Windows USB key that I'll make from within Bootcamp.
Before that, you could boot from a clean windows image and use the startup repair tool within the installer. Are you saying that I will not be able to see/choose a Windows repair DVD or USB drive at startup, as an option? Solved:) so I'll describe what worked for me. Short version - use a Windows Install DVD, but don't do the install. This is similar to what suggested below - thank you!. Boot the system with a Windows 10 Pro 64-bit installation DVD in the external DVD drive, and hold down the OPTION key during boot. A screen will appear where you can choose from which partition/drive to boot: MacOS and Windows (Boot Camp) will appear as options initially, and after a short delay the Windows DVD will appear as a 3rd option.
Choose the DVD option. Wait for a bit until a Windows dialog appears, asking you to choose languages for install - cancel out of this since we don't want to install. For me, this was enough to cause the system to present the normal Windows login screen (I'm not sure if it rebooted when I cancelled the installation, or what - but it was pretty quick). Login and you should be in 'your' Windows OS - i.e. From your Boot Camp partition.
Optionally, check for Windows Updates, check your disks, make a recovery USB key or DVD just in case, enable legacy boot mode menu on startup with F8, backup your data, find your Windows product key, aka do whatever you think might help prevent the 'lockout' in the future. You can also tell Windows to reboot into Safe Mode.
But in my case, after step #4 above, I ejected the DVD, disconnected the DVD drive, and rebooted - and it's working fine. Maybe this will help others:).
